We work with the resident's physicians, family members, and any current care providers to understand the full picture — medical history, current diagnoses, medications, therapy needs, dietary requirements, and personal preferences. We coordinate with each resident's primary care physician, specialists, pharmacists, and therapists on an ongoing basis. When a physician changes a medication, we update the care plan immediately and ensure all caregivers are informed. When lab work is needed, we arrange it. When a specialist appointment is required, we schedule it and, if needed, arrange transportation. We keep detailed records of every interaction, every change, and every observation, creating a comprehensive health history that any healthcare provider can reference.
